Butterbur extract is evidence-supported for migraine prophylaxis. Crude butterbur plant contains hepatotoxic and carcinogenic pyrrolizidine alkaloids (PAs). Only PA-free certified extracts (e.g., Petadolex) should be used. Even with PA-free extracts, cases of hepatotoxicity have been reported, likely from product contamination. Liver function should be monitored with prolonged use. Avoid in liver disease and pregnancy.
